Chronic Neck and Back Pain from Military and Work Injuries

Oct 13, 2025

A 54-year-old male came to our clinic with chronic neck, upper, and lower back pain that had persisted for several years. The pain began during his military service and was later aggravated by a workplace fall that led to hand surgery and a lower back injury.

He reported that the pain made it difficult to perform his job, which involves long periods of standing, and limited his ability to enjoy family time, social activities, and workouts, despite exercising almost every day.

Chief Complaints

  • Neck, Upper Back, and Shoulder Pain: Ongoing for 3+ years, daily discomfort rated up to 9/10 at its worst.
  • Low Back Pain: Chronic since military service, aggravated by prolonged standing and work activity.
  • Stiffness and Fatigue: Wakes up tired and sore, with limited neck mobility and shoulder tension.

Medical & Lifestyle History

  • Past Injuries: Military-related lower back injury and a workplace fall requiring hand surgery.
  • Chronic Conditions: Diagnosed with diabetes.
  • Activity Level: Exercises regularly and works out daily but experienced pain during and after physical activity.
  • Work & Posture: Job involves long periods of standing, lifting and a 20–25 minute daily commute.
  • Sleep Habits: Side sleeper, reports waking up stiff and sore most mornings.

Initial Findings

The initial chiropractic evaluation revealed reduced motion and discomfort in the cervical spine:

  • Flexion: 45° (normal 50°)
  • Extension: 50° (normal 60°)
  • Left Lateral Flexion: 40° (normal 45°)
  • Right Lateral Flexion: 35° (normal 45°)
  • Left Rotation: 65° (normal 80°)
  • Right Rotation: 70° (normal 80°)

Muscle tension was noted across the neck and upper back, with postural issues including forward head carriage and shoulder imbalance. Neurological tests were within normal limits.

Treatment Plan

His treatment plan included a structured, multi-therapy approach:

  • 24 sessions of chiropractic adjustments to restore spinal alignment and mobility
  • 18 sessions of spinal decompression therapy to relieve disc pressure and promote healing
  • 4 sessions of shockwave therapy to address chronic muscle tension and soft tissue restriction

He was also prescribed strengthening and mobility exercises to perform 2–4 times per week, aimed at supporting spinal stability and maintaining postural balance.

Post-Treatment Evaluation

The patient reported gradual but consistent improvement. His pain levels decreased significantly by about 90% and he felt stronger, more relaxed, and able to sleep better.

Post-treatment testing and 3 re-exams later showed restoration toward normal range of motion:

  • Flexion: 50° (normal 50°)
  • Extension: 55° (normal 60°)
  • Left Lateral Flexion: 40° (normal 45°)
  • Right Lateral Flexion: 40° (normal 45°)
  • Left Rotation: 75° (normal 80°)
  • Right Rotation: 70° (normal 80°)

He was able to resume work, physical training, and daily activities with minimal discomfort, reporting noticeable improvements in posture, energy, and overall mobility.

Final Outcome

After completing his personalized corrective care plan, the patient experienced substantial relief and restored function across his neck and back.

His progress demonstrates how a combination of chiropractic adjustments, spinal decompression therapy, and targeted strengthening can effectively address long-standing pain and functional loss from past injuries.
